Step 3

  • Complete and sign the application on pages 3 and 5.
  • Include a letter from your insurance provider detailing your level of coverage.
  • Include receipts or invoices for the costs you have incurred.
  • Provide a copy of your driver’s license.
  • If your are a homeowner, include a copy of your most recent property tax bill.

Step4

Submit the form with copies of any other pertinent information.

If you are a homeowner or a tenant applying for assistance under the Special Provisions for Low-income Households, follow the Provisions for Low-income Households link to determine if you are eligible. Additional information and a separate form will be required.
